ALSNEWS READER SURVEY
Judging from the number of subscribers to ALSNews (now over 500), we believe
this electronic bulletin is indeed an effective and efficient way to keep you,
our readers, up to date on ALS activities. However, we think there is room for
improvement, and thus we are actively soliciting feedback in the form of this
brief survey so we can tailor ALSNews to our audience.
All we ask is that you take one minute (maybe less if you're quick!) to answer
the following questions. Please choose whichever method is easiest for you to
return this survey (for a little vocal challenge, try reading the following
paragraph out loud in one breath):
If you received this survey by electronic mail, you can either
a) Send us an e-mail reply which includes this message, inserting an X
at the beginning of the appropriate lines to mark your answers and
adding your comments; or
b) Send us an e-mail reply with the number of each question and
the letter of your answer(s): 1-A, 2-C,D, etc., plus your comments;
or
c) Print out the survey, circle your answers, add your comments,
and fax back to ALSNews at (510) 486-7696.
If you are reading this on the World Wide Web, you can use options (b) or (c)
above, or copy the survey into an e-mail message and use option (a) above. Our
e-mail address is alsnews@lbl.gov
